A 60-minute comprehensive assessment to initiate your personalized care plan.
All new patients (including established patients who haven’t seen Dr. Hasan in more than 2 years).
Office and virtual visits.
Expect a detailed review of your medical history, outside records, imaging, nutrition status, current concerns and symptoms, and a physical exam. Appropriate testing is ordered at the end of the visit.
Includes a follow up phone call to go over your results and plan the next steps.*

* Your initial consultation includes 30 days of secure portal messaging for follow-up questions. After that, we’ll ask you to schedule a visit or join a membership for ongoing messaging access
Follow up Visit (Non-members)
A 30-minute follow up (pay-per-visit).
This model is appropriate for patients who expect to be seen once or twice a year, without needing services or no communication between the visits.
Office and virtual visits (note the slight difference in pricing between them).
Includes up to one phone call after the visit to discuss labs and imaging results if ordered during the visit.**
Additional services in between appointments will require another visit.
Ongoing portal messaging is not offered, which is a benefit to our subscribed members.
** Note: for those with complex medical problems that require more frequent monitoring and engagement, a membership would be more appropriate.
Concierge Membership
Includes up to 4 visits per year.
Office and virtual visits offered.
Ideal for routine monitoring and management of less complex chronic problems (3 or fewer).
Unlimited communication with Dr. Hasan via Patient Portal & e-mail.
Priority access for appointments within 5 business days.
Covers unlimited prescriptions, filling out forms on your behalf (such as prior authorizations), unlimited testing and interpretation, communication on your behalf (with insurance, other physicians, work, etc).
Basic primary care services for those who don’t have a PCP
Premium Concierge Membership
Includes up to 8 visits per year.
Office, virtual, and phone visits offered.
Ideal for complex problems that require close monitoring and frequent follow ups.
Unlimited communication with Dr. Hasan via Patient Portal, e-mail, mobile phone & text (including on the weekends).
Priority direct access to your physician for urgent queries and same day or next day appointment.
Covers all the services in regular membership
Option to schedule appointments on the weekend (upon request).
Health coaching calls with the doctor once a month (optional)

Direct Access – Members can message the doctor anytime without waiting weeks for an appointment. They also have priority for appointments.
