About non-member visits
Pay-per-visit care is offered at Dr. Hasan's discretion, for stable patients who need 1–2 visits a year. A non-member visit does not include:
- Portal messaging between visits
- Phone calls with Dr. Hasan ($50 per 10 minutes)
- Forms, prior authorizations and appeals ($50 each)
- CGM or pump download and review ($50)
- Prescriptions or refills outside a scheduled visit
A longer 45-minute non-member visit, occasionally needed, is $350.
About membership
- No membership fee is charged for the first 30 days after a paid initial consultation.
- Pay for the year up front and save 10%.
- Prescriptions, refills and test interpretation are included between visits.
- Membership does not replace emergency care. For emergencies, call 911.

Standard member total is 6 months at $130. No membership fee is charged for the first 30 days after a paid initial consultation. Your own costs depend on your care.
Premium membership
Reach Dr. Hasan directly, from anywhere
Premium members reach Dr. Hasan directly through the Spruce Health app and his personal cell phone, including evenings, weekends and holidays for urgent questions. You can share glucose logs, CGM reports, medication photos and lab results, all in one place.
Premium also includes primary care and unlimited visits, as medically needed, so your endocrine and everyday care are managed by one doctor who knows you.
Routine questions are answered during business hours. Spruce is not for emergencies; call 911.
Why we stay small

Dr. Hasan limits the practice to about 300 members. A typical doctor's practice often has 2,000 patients or more. A smaller practice is what makes quick appointments, direct access and longer visits possible.

Which option is right for me?
Dr. Hasan will discuss this with you at your consultation. Pay-per-visit care is offered at his discretion for stable patients seen once or twice a year. Standard suits patients whose conditions need adjustments and support between visits. Premium suits patients with complex or multiple conditions, or who want their primary care and direct access to Dr. Hasan included.
What does the initial consultation include?
A 60-minute evaluation with Dr. Hasan, in office or by secure video, for $400. Before the visit, he reviews your intake forms, past records and outside labs, when available. At the visit, he reviews your history, symptoms and nutrition, and examines you. Afterward, he calls you with your test results and a clear plan, and you have 30 days of support and communication with him.
How many visits does membership include?
Standard includes up to 4 visits a year. Premium includes unlimited visits, as medically needed. Members pay no fee for their included visits.
Can I reach Dr. Hasan between visits?
Standard members use secure patient portal messaging and office calls. Premium members can reach Dr. Hasan through the Spruce Health app and his personal cell phone, including evenings, weekends and holidays for urgent questions; routine questions are answered during business hours. Non-members are asked to schedule a visit, including for prescriptions and refills. For emergencies, call 911.
Do you take insurance?
Chia is a direct-pay practice, so visits and membership are not billed to insurance. We provide an itemized receipt you can submit for possible out-of-network reimbursement. Lab tests and imaging are billed to your insurance as usual.
How do cancellations work?
Members can reschedule or cancel visits flexibly, at no charge. Ending a membership requires 90 days' notice, and the membership fee continues during the notice period. Rejoining later carries a $300 re-enrollment fee. Non-members may reschedule twice a year for a valid reason. Changes within 3 business days of the visit require the full fee in advance, which is non-refundable. Changes made earlier require a $100 non-refundable deposit, credited toward the visit.
